Diabetes Management

Diabetes is a chronic disease that affects approximately 16 million Americans. A diagnosis of Type I or II diabetes can change your life--but doesn't have to diminish your life. Diabetes can be controlled through self-management of three key factors--meal planning, exercise, and medications. Diabetes, once diagnosed, lasts a lifetime and the treatment must last a lifetime too. When it comes to managing diabetes, knowledge is the best medicine.

A team of qualified health care professionals will provide you and your family with the education and counseling you need. Our goal is to help you successfully manage your disease.

Sessions are designed for both individuals who are newly diagnosed and those who have had diabetes for some time but need to learn the latest techniques to self-manage their diabetes. Each program begins with an initial two hour evaluation with a registered dietitian and a certified diabetes educator. Follow-up is continued with either individualized appointments or in a group setting.

Comprehensive education begins with an individualized treatment plan and incorporates:

  • Understanding what diabetes is
  • Customized meal planning
  • Diabetes medications - oral agents or insulin
  • Exercise guidelines
  • Blood glucose monitoring
  • Acute complications
  • Sick Day Management
  • Foot exam and foot care

  • Management of diabetes during pregnancy

Admission to the program is by physician referral only. Throughout the entire program, the staff coordinates your care with your physician. The Medical Center of Plano's diabetes education program has been awarded Recognition by the American Diabetes Association.
